SKILLED (MEDICAL) SERVICES
All skilled, medical services will be available under the direction
of a licensed physician with an established physician's plan of
care, (POC), for those patients who require skilled nursing, home
health aide and physical therapy services.
The RN, LPN, and home health aide, may provide visits twenty-four (24)
hours per day up to seven (7) days per week. Scheduled two (2) hour
minimum visits are also available twenty-four (24) hours per day up
to seven (7) days per week. Services of a physical Therapist will be
scheduled by the therapy provider on an individual basis, according
to the physician's plan of care (POC).
ATTENDANT CARE (NON MEDICAL SERVICES)
Services will be available for clients who require non-medical
care. A written service plan agreement will be established and
directed by
the designated staff member of the agency. Personal care attendants
(PCA' 5), homemakers and sitters may provide visits twenty-four
(24) hours per day up to seven (7) days per week. Scheduled minimum
two
(2) Hour visits are also available twenty-four hours per day
up to seven (7) days per week.
OPERATING HOURS:
Normal operating hours will be from 8:00 AM - 4:00 PM, Monday - Friday.
The office will close at 4:00 PM; however, extended and emergency
services will be under the supervision of an "on call" nurse.
Telephone calls received between 4:00 PM Friday till 8:00 AM Monday
will be received by the answering service and forwarded to the "on
call" nurse.
Telephone calls received during holidays, when the office is closed,
will be received by the answering service and forwarded to the "on
call" nurse.
The above also applies for non-medical clients with one exception.
In case the client experiences a medical emergency the client is
instructed to call 911.
For all other inquiries received after hours, calls will be received
by the answering service and forwarded to the designated agency personnel.
